Report an Injury
To report an injury, you can print out the PDF form and submit it to The Health Plan Managed Workers' Compensation Department, or you can call The Health Plan directly at 1.888.847.7810 ext. 6109. You can also fill out the form and fax the completed form to 1.877.847.6927.
